The 12th Combat Aviation Group was initially formed by the U.S. Army in April 1964 as the U.S. Aviation Group (Provisional) and assumed command in May 1965 of the 3/17th CAV, 11th, 145th , 210th, 222nd, 269th, and the 308th Combat Aviation Battalions plus the 765th Transportation Battalion. The U.S. Aviation Group was re-designated as the 12th Combat Aviation Group on August 28, 1965. The 12th Group made their base at Tan Son Nhut (Saigon) until June 29, 1966 when the group relocated to Long Binh as part of the newly formed 1st Aviation Brigade. One of 12th Combat Aviation Group largest early campaigns was in support of Operation Attleboro in October and November 1966 during which the group flew over 46,000 troops. In January and February 1967 they supported Operations Cedar Falls and Junction City on a similar scale.
